We are Minnesota’s only county-owned, state-certified health maintenance organization. As a managed care organization, our outstanding, collaborative team takes a holistic approach to health care by integrating traditional medical services, behavioral health services, and other county and community services that address the social influencers of health.

We serve Hennepin County adults, families and children enrolled in Minnesota Health Care Programs. We offer three health plan products:
